Baseline Risk Factors (Category A)

Check any that apply to determine if a baseline test is needed.

Note: A history of incarceration typically warrants baseline testing plus a follow-up test ~6 months after release to cover the incubation period. Ongoing periodic testing (Category B or higher) is generally only needed if high-risk behaviours associated with incarceration are continuing post-release, or if other separate risk factors are present. This needs assessment by a healthcare provider.
Note: This typically warrants baseline testing plus a follow-up test ~6 months after moving out to cover the incubation period. Ongoing testing is usually not needed unless other risk factors are present.
Note: This history typically warrants baseline testing. Ongoing periodic testing (Category C or D) is only needed if this behaviour is current and meets the partner number thresholds below. This needs assessment by a healthcare provider.
Note: This typically warrants baseline testing around the time of cure confirmation to check for HBV co-infection acquired previously. Ongoing testing is usually only needed if the risk behaviours that led to HCV are still present.
Note: A recent STI diagnosis serves as a marker for recent sexual activity that could also have exposed you to HBV. A baseline HBV test is recommended. Ongoing testing depends on whether high-risk sexual behaviours are continuing - see Categories C & D.
Ongoing Risk Factors (Category B - Annual Testing)

Check any that apply IF you are susceptible (unvaccinated, non-responder, or immunity unconfirmed).

Note: While the number of partners increases potential exposure frequency, consistent use of risk reduction strategies significantly lowers the per-encounter risk. Annual testing is suggested, but individual assessment with a provider is crucial.
Note: Even with one partner, specific high-risk practices warrant ongoing monitoring. Annual testing is suggested, but individual assessment with a provider is crucial.
Ongoing Risk Factors (Category C - 6-Monthly Testing)

Check any that apply IF you are susceptible (unvaccinated, non-responder, or immunity unconfirmed).

Note: HIV co-infection significantly increases risk. Six-monthly testing provides regular monitoring for susceptible individuals. Frequency might be adjusted (e.g., annually for very stable patients with no other risks, or more often if other high risks are present) based on guidance from a healthcare provider considering HIV control, HBV vaccination status/response, etc.
Ongoing Risk Factors (Category D - 3-Monthly Testing)

Check any that apply IF you are susceptible (unvaccinated, non-responder, or immunity unconfirmed).
